Waiver and Release of Liability Maranatha Bible Church - In consideration of my child’s participation in Cabin Fever Festival, hereinafter referred to as "the Activity," I agree as follows: I, on behalf of myself, my family, my personal representatives, relatives, and assigns, hereby voluntarily agree to release, waive, forever discharge, hold harmless, defend and indemnify Maranatha Bible Church and its volunteers, members, agents officers, elders, employees, invitees (hereinafter collectively referred to as "Maranatha") from any and all claims, actions, causes of action, damages, expenses or losses from bodily injury, property loss or damage, wrongful death, loss of services and consortium, or any other loss or damage of any kind whatsoever (hereinafter referred to as "Claims"), which may arise out of my child’s participation in the Activity, the use of the building and equipment whether or not associated with the Activity, any injury or damage as provided herein to anyone accompanying me to or in the Activity and transportation to or from the Activity. I do hereby declare myself and my child to be medically able to participate in the Activity. I understand that there are risks which may include, but are not limited to, disabling injury and/or death involved in the Activity, and I agree that my child and I will familiarize ourselves with all equipment, facilities, rules, and physical demands related to the Activity, and we hereby assume the risks of the Activity. I specifically understand and agree that I am assuming all risks and dangers associated with the Activity and all responsibility for Claims, whether caused by Maranatha, the negligence of other participants, the negligence of any others, accidents or other causes, and that I am specifically releasing, discharging, holding harmless, agreeing to defend and indemnify and waiving any claims that I may have presently or in the future against Maranatha. Additionally, the attendee grants Maranatha Bible Church permission to email them regarding the Cabin Fever Festival. If the attendee does not wish to be photographed, they will notify registration personnel upon arrival. I HAVE READ THE ABOVE WAIVER AND RELEASE OF LIABILITY FORM AND BY SIGNING IT AGREE THAT IT IS MY AND MY CHILD’S INTENTION TO ENGAGE IN THE ACTIVITY, ASSUMING ALL RISKS WHATSOEVER, KNOWN AND UNKNOWN, AND TO WAIVE, EXEMPT, RELIEVE AND HOLD HARMLESS AND AGREE TO DEFEND AND INDEMNIFY MARANATHA BIBLE CHURCH FROM ANY AND ALL CLAIMS.
